Responsibilities

Responsible for assisting the clinical operation by providing telephonic case management as well as quality, patient satisfaction and process flow initiatives and reports to the Manager of Clinical Practice.

 

  • Collaborates with specialists, other physicians, and professionals at local healthcare facilities and in the community, to provide home health services to patients in need of skilled nursing care. Coordinates care by ensuring effective communication and completion of treatments/procedures. 
  • Utilizes the nursing process (assessing, planning & implementing care) to provide nursing visits &/or manage a nursing case manager caseload, as needed. Provides comprehensive care and teaching to patients and families regarding the individualized plan of care. Applies clinical knowledge and skills (i.e., administration of medications, treatments, procedures, etc.) to safely provide direct patient care in a professional, caring, culturally- sensitive, and cost efficient manner; ensuring appropriate clinical coordination and support of patients and families. 
  • Assist client care coordinators in prioritizing patients in scheduling visits for case managers, through clinical chart/ documentation review and triage, ensuring timely starts of care. 
  • Functions as the patient advocate in coordinating patient care; including performing crisis intervention when required. Collaborates and confers with community agencies to identify resources that meet the patient's needs. Refers patients to community resources, as needed. 
  • Attends patient care conferences and staff meetings as needed. Communicates the patient's special needs to appropriate team members. 
  • Documents assessment and reassessment data, patient care activities, interventions, patient's responses, patient and family education, and discharge plans according to the plans of care, policies and procedures, etc., within predetermined timeframes. 
  • Identifies outcomes and patient's response to treatments and procedures to include the effectiveness of interventions and initiatives (to include pain management). Reports all significant or unexpected outcomes of interventions to the patient's physician. 
  • Identifies learning needs. Recognizes appropriate educational content and methodologies required to meet the patient's and/or family member's specific educational needs. Identifies learning readiness. Initiates learning activities. 
  • Delegates patient-specific tasks and activities to ancillary personnel based on competencies, regulatory, agency, and program-specific policies, procedures, and guidelines as required. Collaborates with support staff regarding delegated tasks &/or activity to ascertain if care was provided according to the care plan. 
  • Serves as a resource to the nursing staff, physicians, and other health care team members related to home care. Mentors current and new staff, when needed; participating in educational programs and workstreams. 
  • Run and review productivity, and clinical reports as well as conduct periodic audits of patient records for QAPI. 
  • Evaluates patient care practice in relation to individual team/patient satisfaction targets, and follow-up as needed. 
  • Work in EMR to review admissions and discharges, ensuring appropriate clinical documentation and OASIS accuracy if needed. 
  • Maintains personal growth and development by attending educational programs and workshops. Reviews current literature to maintain competencies and expertise in clinical practice. 
  • Other duties and/or projects as assigned. 
  • Adheres to HMH Organizational competencies and standards of behavior. 
  • Lifts a minimum of 50 lbs, pushes and pulls a minimum of 50 lbs and stands a minimum of 4 hours a day.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Associate's Degree in Nursing or Diploma Certificate in Nursing. 
  • Minimum of two (2) or more years RN Experience. 
  • Minimum of three (3) years certified home care experience. 
  • Excellent written and verbal communication skills. 
  • Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms. 

 

Education, Knowledge, Skills and Abilities Preferred:

  • OASIS certified. 
  • BSN. 

 

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License. 
  • BLS AHA Basic Health Care Life Support HCP Certification. 

Compensation
Minimum rate of $92,144.00 Annually

  • Geographic Location: Cost of living and market rates for the specific location.
  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
