Overview

The Structural Heart APN Coordinator facilitates the policies, procedures, protocols and standards for the structural heart program. Participates in program and budgetary planning that involves cost containment, procedure costing, and billing questions for the program. The coordinator communicates with the physician/manager/director advising him/her of operations status, pending problems, and to collaborate on projects. The coordinator will oversee the operations of the structural heart program which includes but not limited to TAVI, Mitral Clip, Watchman, and other structural heart devices. The Structural Heart APN coordinator is responsible for providing clinical leadership, expertise, collaboration, consultation and mentorship to promote evidence based nursing practice. The APN supports organizational goals. The APN works with the Chairman/Medical Director(s)/Collaborative Practice Physician(s) for designated specialty, and/or Service Administration to achieve optimal standards of care with measurable identified outcomes.

Responsibilities

1. Practices as an autonomous and collaborative member of the health care team, demonstrating clinical expertise, differentiating between normal findings and those needing treatment, referral and or consultation. 2. Works as a liaison between Program Directors, Departments, Hospital, and other outside entities. Demonstrates patient advocacy, which assures patients` rights to privacy and confidentiality. Helps in coordination of program promotion. Ensures that referrals and authorizations cover ordered treatments and assists in procuring test and treatment authorizations and results. 3. Actively participates in multidisciplinary rounds. 4. Facilitates the out-patient clinical practice organizing the patient follow ups and consultations. 5. Responsible for the data collection and validation of the TVT national database registry. 6. Implements quality program initiatives to improve the national TVT benchmark rankings. 7. Facilitates meetings routinely with program personnel to disseminate information; demonstrates knowledge of standards and regulations of applicable regulating and accrediting agencies and ensures their enforcement and compliance. 8. Provides patient and family education and service support functions such as coordinating and conducting new patient education, preparing patients for procedures, treatments and special interventions, assisting patients to understand treatment options, and attending patient care conferences. 9. Provides orientation to new patients and families including education on planned treatments, appointment and procedure scheduling, general facilities and other available patient/family resources. Provides ongoing education to patients and families regarding planned new treatments or diagnostic procedures. Supports clinic staff with education activities as needed to ensure effective patient care. 10. Effectively communicates with physicians, patients, families, and staff regarding scheduling, procedural, clinical questions. Maintains a database of all patients seen in the Valve Program. Assists with data collection involved for registry maintenance purposes. 11. Facilitates the enrollment and screening for Structural Heart Research studies in collaboration with the HVH Research team. 12. Demonstrates advanced assessment skills, within clinical specialty, providing evidence-based treatment interventions, to achieve positive outcomes for patients and families. 13. Formulates a health treatment plan based upon pertinent health appraisals which include labs, diagnostic tests/procedures, and history/physicals. 14. Participates in the development and recommendation of evidence-based practice identifying the goal and desired measurable outcomes, which may include clinical, financial, process/system and patient/family satisfaction. 16. Adheres to the standards identified in the Medical Center's Organizational Competencies.

Qualifications

Education, Knowledge, Skills and Abilities Required: 1. Master's degree. 2. Certified as a Nurse Practitioner, Clinical Nurse Specialist or Advanced Practice Nurse by a national accrediting organization, which is approved by the board. P.L. 1991, Chapter 377/subchapter 7. 3. One year Clinical experience in structural heart. 4. Excellent communication skills. 5. Collaborative agreement with physician(s) actively on staff who agree to act as the Collaborating Physician(s) of Record as per the APN Act and Regulations, if appropriate to role. Education, Knowledge, Skills and Abilities Preferred: Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License and Advanced Practice Nurse Cetification (w/ Master's Degree) and Controlled Drug Substance and AHA Basic Health Care Life Support HCP Certification. 

Compensation
Minimum rate of $124,571.20 Annually

Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization. Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered. Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
