The Healthstart Coordinator is responsible for providing the highest standard of nursing care for the all maternity patients of the Family Medicine Center. The Coordinator supports and implements the philosophy, goals and standards of JFK University Medical Center and the Nursing Department of the Family Medicine Center as well as those of the Healthstart Program. Active member in a patient centered-team approach in the care of the patient and families. Works collaboratively with all team members in achieving shared goals with a consistent focus on quality care. Participates in quality improvement activities including data analysis, implementing recommendations, evaluating changes and outcomes.
Responsibilities
A day in the life of a Healthstart Coordinator at Hackensack Meridian Health includes:
- Implements and follows through on Healthstart standing orders and guidelines. Assigns the primary care physician and schedules follow up appointments into the future. Coordinates and schedules any diagnostic tests based on protocols. Informs and educates the patient on the details and reasons related to the testing; and provides patients with educational forms and information such as required. As the Case Coordinator, appropriately refers patients to the Social Worker and Dietician.
- Primary responsibility and focus is the OB patient who seeks prenatal care at Family Medicine. Possess sound nursing skills, critical thinking, and strong clinical judgment with an emphasis on OB. In the initial patient interview and assessment, collects pertinent health, medical and social history; and other information from the patient, family, and significant others including allergies, vital signs, pain assessment, smoking status, any other related data as required. Documents in the patient record utilizing hospital approved abbreviations and adhering to JFK documentation standards. Plays a crucial role in informing any and all members of the health care team, of any issues and problems which could impact the care of the patient.
- Responsible for the oversight and adherence to all Healthstart guidelines and policies. Strong focus on patient satisfaction and safety. Assumes personal responsibility for professional growth by the completion of all required hospital educational programs, and attendance at educational programs. Plays an important role and is an active participant in the overall resident educational process. Acts as a facilitator in the event patient's expectations are not met and attempts to resolve the issues.
- Utilizing sound nursing and clinical judgment, supervises and assists other members of the health care team in the delivery of OB patient care within the framework of the Patient Centered Medical Home model. Preliminarily provides information and educates patients on the Centering Pregnancy Program. Provides related information to monitor the overall status of all the OB's as it relates to statistics and financial status of the practice
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- Graduate of an accredited school of professional nursing.
- Minimum of one to two years maternal/child experience. Able to multitask and work in a team.
- Demonstrates effective communication skills.
Education, Knowledge, Skills and Abilities Preferred:
- BSN.
- Knowledge of Centering pregnancy.
- Computer skills.
Licenses and Certifications Required:
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification.
Licenses and Certifications Preferred:
- Advanced Cardiac Life Support Certification.
- Pediatric Advanced Life Support Certification.

Compensation
Minimum rate of $38.27 Hourly
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.
The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate. - Labor Market Data: Compensation is benchmarked against market data to ensure competitiveness.
- Experience: Years of relevant work experience.

- Skills: Demonstrated proficiency in relevant skills and competencies.
- Geographic Location: Cost of living and market rates for the specific location.
- Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
