The Community Outreach Coordinator II is responsible for planning, coordinating and implementing Hackensack Meridian Health's (HMH) Community Health Needs Assessment (CHNA) as well as developing strategies to address identified community health needs and in support of Hackensack Meridian Health's strategic imperatives and centers of excellence.
Responsibilities
A day in the life of a Community Outreach Coordinator II at Hackensack Meridian Health includes:
- Assist in coordinating HMH's network-wide CHNA process for all hospitals. Provide project management support to senior leader in all phases of CHNA development including strategic planning, vendor selection and management, steering committee, community benefit subject matter expert, CHIP development, as well as ongoing management and reporting.
- Using the CHIP as a roadmap, develop community outreach plan for assigned territory. Collaborate with clinical outreach coordinators to identify evidence-based programs and resources to address identified health needs. Determine appropriate goals, outcomes and impact. May assist with the implementation of outreach activities. Ensure cultural competence and health literacy needs are addressed as needed.
- Implement assigned outreach activities according to the community outreach plan. Ensure all logistics related to health screenings, health fairs, education programs and special events are completed in a timely, efficient and accurate manner.
- Identify, develop and maintain relationships with key community contacts (ie. health department, senior centers, faith-based organizations, community groups, employers, etc) who share the mission for improving health within the Hackensack Meridian Health communities we serve and that support the CHIP for each hospital as well as address Social Determinants of Health (SDOH).
- Represent Hackensack Meridian Health Community Outreach department on boards, coalitions and committees that are strategically aligned with the community benefit plan. Reports and communicates meeting information and follow up items.
- Collaborate with HMH foundation and community organizations to identify grant opportunities to further the mission of improving the health of the communities we serve. Define grant purpose, objectives, goals, resources and budget.
- Accurately collects data and inputs information into outreach database or other system on a regular basis. Completes timely, accurate and complete event summary forms as per protocol to support 501r compliance reporting.
- Conduct health impact assessment to determine success of outreach programs. Identify measures of success and data collection plan. Develop policies to ensure consistency of delivery and measurement.
- Prepare monthly management report summarizing outreach activities and outcomes. Provides program attendance and evaluation forms for events.
- Assists in mentoring and orienting new team members.
- Represent HMH at (non-marketing) community events. Can articulate the mission, vision and values of Hackensack Meridian Health as well as programs, services and resources available through the health system. As needed, transport equipment and supplies, set-up and break-down of exhibits/tables, dissemination of health information and community resources, registration of guests.
- Coordinate with call center to ensure representatives are informed of current programs.
- Successfully completes yearly department competencies as assigned. Completes Health Stream modules on time as assigned.
- Independently develops and evaluates a personal continuing education and professional development plan.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- Bachelor's degree.
- Excellent organization and communication skills.
- 5+ years experience working in the community setting with diverse populations.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Master's in public health or related field
- Bi-lingual (Spanish) a plus
- CHES a plus
Licenses and Certifications Required:
- Current Driver's License from a USA state.

Compensation
Minimum rate of $61,110.40 Annually
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.
The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate. The base compensation determined at the time of the offer may be different than the posted rate of pay based on a number of non-discriminatory factors, including but not limited to:
- Labor Market Data: Compensation is benchmarked against market data to ensure competitiveness.
- Experience: Years of relevant work experience.
- Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
- Skills: Demonstrated proficiency in relevant skills and competencies.
- Geographic Location: Cost of living and market rates for the specific location.
- Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
