Visiting Health Services of New Jersey (VHSNJ), a partnership between Hackensack Meridian Health and St. Joseph's Health, provides a comprehensive continuum of home health care and hospice services, support, and resources to patients and families in Passaic, Bergen, and parts of Morris counties. The VHSNJ multidisciplinary clinical and support teams provide high quality care to patients where they most want to be-in their homes.

Responsibilities

The primary role of the Clinical Liaison, Hospice is to connect healthcare facilities with hospice services by performing face to face encounters with patients and families contemplating hospice services. Responsibilities will include building relationships with referral sources, provide hospice education to patients and families, perform nursing assessments to determine hospice eligibility, coordinate hospice admission process and collaborate with the hospice team.

 

  • Referral management: Respond timely to referral requests from facilities, hospitals, and other health care providers.
  • Patient Assessment: Conduct comprehensive nursing assessments to determine hospice eligibility per diagnosis and qualifying criteria.
  • Education: Educate patients, families and healthcare professionals about the hospice philosophy, services, benefits and end of life process.
  • Admission Coordination: Initiate and facilitate the admission process, inclusive of obtaining necessary paperwork and payor information. Perform clinical, admission assessment as needed/necessary.
  • Relationship building: Develop and maintain strong, positive relationships with case managers, discharge planners, physicians, and other key stakeholders.
  • Collaboration: Work closely with the hospice clinical team, scheduler and other team members to ensure smooth transition to hospice services.
  • Patient Advocacy: Act as a patient and family advocate throughout the referral and admission process, ensuring their needs and preferences are addressed.
  • Marketing and Outreach: Participate in marketing activities and collaborate with the sales team.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.
  • Lifts a minimum of 50lbs, pushes and pulls a minimum of 50 lbs and stands a minimum of 6-8 hours per day.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Associates degree in Nursing.
  • Minimum 5 years experience in an acute or post acute care setting.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  • Hospice experience
  • BSN

 

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.
  • BLS AHA Basic Health Care Life Support HCP Certification.

 

Licenses and Certifications Preferred:

  • Certified Hospice and Palliative Nurse.

Compensation
Minimum rate of $111,924.80 Annually

The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

  • Experience: Years of relevant work experience.
  • Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
  • Skills: Demonstrated proficiency in relevant skills and competencies.
  • Geographic Location: Cost of living and market rates for the specific location.
  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.

In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
