Responsibilities

The Manager of Quality and Data Analytics is a strategic leader responsible for driving the quality and performance improvement initiatives across all home health and hospice operations. This role involves the comprehensive management of the Quality Assurance and Performance Improvement (QAPI) program, ensuring regulatory compliance, and leveraging data to enhance clinical outcomes and patient satisfaction. The manager will oversee all data collection and analysis related to quality measures, infection control, and value-based purchasing, translating complex data into actionable strategies. By collaborating with operational leaders and clinical teams, this individual will optimize clinical information systems, guide survey readiness, and ensure the highest standards of patient care and documentation integrity.

 

  • QAPI Program Management and Oversight: Directs and supports all aspects of the agency's QAPI program, including the annual update of materials, organization of binders for survey presentation, and leadership of quarterly meetings. Guides teams in the development, implementation, and presentation of Performance Improvement (PI) projects, ensuring data is analyzed and presented effectively to drive meaningful change.
  • Data Analytics and Strategic Reporting: Manages the collection, analysis, and maintenance of all quality and patient satisfaction data from sources such as CMS and SHP. Develops and maintains spreadsheets and dashboards to track key performance indicators for home health (including Home Health Value-Based Purchasing) and hospice.  Provides data-driven insights and prepares reports for leadership and board meetings to inform strategic decisions.
  • Regulatory Compliance and Survey Readiness: Ensures the agency is in a constant state of readiness for Department of Health and Joint Commission surveys. This includes maintaining and updating survey preparation resource guides, developing survey-ready reports within the electronic health record, and assisting surveyors by navigating clinical charts and providing necessary documentation.
  • Infection Prevention and Control Leadership: As the credentialed Infection Preventionist, this manager monitors and analyzes monthly infection data across all operations. They are responsible for organizing and conducting quarterly Infection Prevention and Control meetings and collaborating with leadership to devise and implement new strategies to mitigate infection risks.
  • Health Information System Optimization: Collaborates with Epic analysts and operational leaders to optimize the electronic health record platform. Focuses on enhancing clinician documentation efficiency while ensuring full compliance with all regulatory requirements, thereby improving data integrity and workflow.
  • Strategic Collaboration on Quality Initiatives: Works closely with operational leaders and specialized clinical staff, such as the Virtual Wound Nurse Coordinator, to brainstorm, develop, and implement innovative strategies aimed at improving quality measures and patient satisfaction scores. This includes collecting and analyzing data for special projects and presenting findings to promote best practices.
  • Collaborative Systems Optimization: Partners with operational leaders and Epic analysts to optimize the Epic platform. This collaboration focuses on enhancing clinician documentation efficiency while ensuring full compliance with all regulatory requirements, and exploring new technologies to improve the educational experience.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.
  • Lifts a minimum of 10 pounds, pushes and pulls a minimum of 10 pounds and stands a minimum of 120 minutes a day.

Qualifications


Education, Knowledge, Skills and Abilities Required:

  • Bachelor's degree in Nursing.
  • Minimum of 5 years of experience in health care.
  • Excellent clinical informatics as well as written and oral communication skills with ability to present  quality outcomes at various leadership level meetings within the organization.
  • Ability to work on multiple projects concurrently in a fast paced, timeline driven environment.
  • Strong planning and organizational skills with the ability to work independently, multi-task, and prioritize responsibilities.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.


Education, Knowledge, Skills and Abilities Preferred:

  • Master's degree (MSN, MBA, MPH).
  • Experience in Clinical Informatics and Clinical Transformation work.
  • Experience in Home Health/Hospice.
  • Performance improvement experience.
  • Experience with leading quality and outcomes work.
  • Understanding of clinical regulatory standards.


Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.

Compensation
Minimum rate of $111,924.80 Annually

HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

  • Experience: Years of relevant work experience.
  • Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
  • Skills: Demonstrated proficiency in relevant skills and competencies.
  • Geographic Location: Cost of living and market rates for the specific location.
  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
