In conjunction with the multidisciplinary team the Nurse Navigator is responsible for the coordination and management of patient navigation through the patient's behavioral health care while at Raritan Bay Medical Center. The position serves as the first-line liaison between the patient, the hospital, and the appropriate healthcare discipline and has primary responsibility for ensuring the goals of the program are met. The nurse navigator's responsibilities include educating patients about their diseases and treatments, translating medical jargon into readily accessible terms, and helping patients to overcome any barriers to healthcare that they may face. You should also bolster patients' and families' emotional resources.

Responsibilities

  • Ensures that the systems and programs of our behavioral health services function effectively and efficiently.
  • Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
  • Educates patients and/or families about medical issues, and use of prescribed medical treatments and/or medications.
  • Serve as a resource to dual diagnosis behavioral health patients:
    • a. Providing resources for patients seeking reliable information.
    • b. Referring patients to Social Workers or Counselors, as needed.
    • c. Answering questions or liaising with doctors to address questions and concerns.
    • d. Providing emotional support and empowerment to patients and their loved ones.
    • e. Assess and screen incoming patients
    • f. Develop an individualized treatment plan for each patient
    • g. Attend multidisciplinary treatment plan conferences for assigned patients and work collaboratively with members of treatment team to achieve identified goals
    • h. Administer medication and provide pain management
    • i. Inform patients, family members and loved ones about the risks of substance use and options for treatment
    • j. Positively support patients' plans for continuing recovery
    • k. Work with physicians, residents and fellows to provide consultation services on patients throughout the institution
  • Maintains legible, accurate, and confidential medical records.
  • Provide for assistance with implementation of recommendations; initiate and support resolution of issues and conflicts in order to meet program goals.
  • Demonstrate and facilitate excellent customer service.
  • Perform other related duties as assigned.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Master's degree in Nursing.
  • Five years of experience in Behavioral Health with at least three years working with dual diagnosis patients.
  • Receipt of a nurse navigator certificate or similar is preferred.
  • Extensive experience working with dual diagnosis patients.
  • Sound knowledge of the healthcare system and where to find reliable information.
  • Excellent verbal and written communication skills.
  • Openness towards different cultures, beliefs, and traditions.
  • Sensitivity, discretion, and respect.
  • Effective organizational, planning, negotiating, coaching, and conceptual thinking skills. Excellent oral and written communication skills.
  • Interpersonal and problem-solving skills necessary to interact with executives and staff.
  • Ability to clearly direct and motivate individuals.

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.

 

Licenses and Certifications Preferred:

  • Nurse Navigator Certification.

Compensation
Starting at $106,329.60 Annually

HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate.   • Labor Market Data: Compensation is benchmarked against market data to ensure competitiveness.
  • Experience: Years of relevant work experience.
  • Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
  • Skills: Demonstrated proficiency in relevant skills and competencies.
  • Geographic Location: Cost of living and market rates for the specific location.
  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.

In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
