In conjunction with the multidisciplinary team the Nurse Navigator is responsible for the coordination and management of patient navigation through the Human Motion Institute/Institute for Weight Loss. The position serves as the first-line liaison between the patient, the hospital, and the appropriate healthcare discipline and has primary responsibility for ensuring the goals of the program are met

Responsibilities

A day in the life of a the Nurse Navigator at Hackensack Meridian Health consists of:

 

  • Ensures that the Human Motion Institute/Institute for Weight Loss systems and programs function effectively and efficiently
  • Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral
  • Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications
  • Maintains legible, accurate, and confidential medical records
  • Participate in development of long-range plans for growth of hospital's Human Motion Institute
  • Serve as resource to hospital staff
  • Respond to questions and concerns appropriately and in a timely manner
  • Provides back up support for difficult projects or assignments
  • Provide instruction, encouragement and support as needed
  • Serve as a resource to Human Motion Institute patients
  • Respond to questions and concerns in an appropriate and timely manner
  • Assists patients navigating through the system to the appropriate discipline timely and efficiently
  • Assists with referrals as needed
  • Assist in developing expectations for interdisciplinary teams and assist with direction/mechanisms for expectations to be met
  • Provide for assistance with implementation of recommendations; initiate and support resolution of issues and conflicts in order to meet program goals
  • Participate in marketing programs to enhance growth
  • In conjunction with other team members, determine referral source targets, identify needs of the referral sources and their patients and facilitate referral source education and relationship building
  • Demonstrate and facilitate excellent customer service
  • Maintain on going communication vehicles to update key physicians, facilitate key issues / barriers and support
  • Other duties and/or projects as assigned
  • Adheres to HMH Organizational competencies and standards of behavior

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Graduate of an NLN/AACN accredited program in Nursing
  • Bachelors Degree or equivalent years of related specialty experience
  • Five or more years of experience in the health care industry
  • Licensed and currently licensed as a professional nurse in the State of New Jersey
  • Excellent written and verbal communication skills
  • Proficient computer skills including but not limited to Microsoft Office and Google Suite platforms

Education, Knowledge, Skills and Abilities Preferred:

  • Masters degree in Nursing.

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License

Compensation
Starting at $53,164.80 Annually

HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate.   • Labor Market Data: Compensation is benchmarked against market data to ensure competitiveness.
  • Experience: Years of relevant work experience.

  • Skills: Demonstrated proficiency in relevant skills and competencies.
  • Geographic Location: Cost of living and market rates for the specific location.
  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.

In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
