A Senior Practice Outreach Coordinator will function as a peer leader and is an experienced Practice Outreach Coordinator (POC) who is at the expert level of both workflow knowledge and skill set as a POC.The role will function to onboard and train new Practice Outreach Coordinators, evaluating and ensuring their competency. This role will also serve as an ongoing mentor of the Practice Outreach Coordinator team to optimize workflows and outreach to patients to close gaps in care. The Senior Practice Outreach Coordinator will have a broad scope in mentoring and supporting the Practice Outreach Coordinator team with workflow optimization and new projects as well as supporting new practice acquisitions. Coaches the team on Team Steps for physician practices, motivational interviewing, relationship building and team building.

Responsibilities

A day in the life of a Senior Practice Outreach Coordinator at Hackensack Meridian Health includes:

  • Subject matter expert with Quality Measure Management (HEDIS, MIPS, and ACO).
  • Support the Practice Outreach Coordinator team with education for new projects and workflows.
  • Act as mentor for Practice Outreach Coordinators as related to gaps in care workflow optimizations and efficiencies to improve quality.
  • Holds practice assignments and conducts proactive outreach to patients to support lab/image/referral tracking, gaps in care and other quality measure improvement efforts.
  • Effective and compassionate communication with patients and their significant others to plan, implement and follow up on gaps in care.
  • Frequent telephonic interactions with patients and their significant other to close gaps in care.
  • Coordination with other members of the care team, Providers on the care team and consultants to close gaps in care and update EMR.
  • Acts as a liaison between patients, primary care practices and population health and Quality teams in the provision of care.
  • Facilitates partnerships with community partners and resources to assist patients in closing gaps in care.
  • Uses evidence-based coaching techniques to help patients design realistic health goals, implement positive lifestyle changes and achieve desired plan in support of a healthier lifestyle and disease state.
  • Works as part of the interdisciplinary team to meet the goals of the Practice, the provider and the patient. Will support the population health and transformation initiatives as assigned.
  • Provides quality customer service- follows HIPAA rules/regulations.
  • As a member of the patient care team, documents in the EMR, the patient interactions and care planning activities as directed by the Provider and care team.
  • Identifies and appropriately escalates patient related issues to the Provider when required.
  • Utilizes data to better understand tailor interventions to the patient populations.
  • Attends/presents status updates at meetings.
  • Supports the Transformation efforts as needed.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Minimum of 2+ years of experience as a Practice Outreach Coordinator on the Ambulatory Quality and Patient Safety team.
  • Associates degree in Healthcare related field, or two years relevant experience with health care or medical practice.
  • Ability to work on multiple projects concurrently in a fast paced, timeline driven environment.
  • Strong planning and organizational skills with the ability to work independently, multi-task, and prioritize responsibilities.
  • Proficiency in Electronic Medical Record required.
  • Must have reliable transportation as travel is required. May need to travel to any location within the Hackensack Meridian Health network depending on organizational need.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

 

Education, Knowledge, Skills and Abilities Preferred:

  • Bachelor's Degree is preferred with focus on Health Care administration, principles of management, Population Health, Health Care Education, Community Health, Health Information Management, or Information Technology.

Licenses and Certifications Required:

  • Valid Driver's License from a USA state.

Licenses and Certifications Preferred:

  • Certified Medical Assistant.

Compensation
Starting at $55,556.80 Annually

HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
