The Supervisor of Social Determinants of Health (SDOH) Data plays a critical role in advancing Hackensack Meridian Health's commitment to addressing the social factors that impact patient health and well-being. This multifaceted position extends beyond direct patient care, encompassing responsibilities related to regulatory compliance, data management, technology integration, interdisciplinary collaboration, and data analysis for the Community Health Worker (CHW) program.

The role is hybrid, with some onsite network responsibilities in Edison, NJ as well as the hospital facilities and medical groups. 

Responsibilities

A day in the life of a Supervisor of Social Determinants of Health (SDOH) Data at Hackensack Meridian Health includes:

  • SDOH Regulatory Compliance: a. Ensure compliance with HHS regulations on SDOH screening. b. Develop and implement policies and procedures for SDOH data collection, analysis, and reporting. c. Ensure integration of SDOH screening into Community Health Worker (CHW) programs workflows.
  • EPIC SDOH Module Utilization: a. Train staff on the effective use of the EPIC SDOH module. b. Identify and document social determinants of health for patients within the electronic health record system. c. Collaborate with IT/DTS to ensure optimal functionality, troubleshoot issues, and implement updates.
  • Unite Us' Referral Platform Management: a. Ensure proper integration of the `Unite Us' platform with other digital systems. Serve as the liaison between the Unite Us platform and the IT/DTS team b. Optimize data exchange and interoperability between Unite Us and the electronic health record system. c. Build and maintain relationships with community-based partners to facilitate referrals and resource utilization.
  • Database Management: a. Oversee the management of the SDOH database, ensuring accurate and up-to-date information on community resources, social services, and patient interventions. b. Work closely with IT/DTS to implement data security measures and privacy protocols in compliance with healthcare regulations.
  • IT/Digital Technology Integration: a. Collaborate with IT/DTS to integrate SDOH data seamlessly into existing digital health infrastructure. b. Participate in the development and enhancement of digital tools and applications that support SDOH initiatives and streamline data collection and reporting. 
  • Interdisciplinary Collaboration: a. Facilitate communication between the SDOH team and IT/digital technology services teams to address technical challenges, implement system improvements, and enhance user experience. b. Facilitate communication between the SDOH team and clinical and allied health services teams to address patient needs and improve care coordination. c. Work collaboratively to develop and maintain standardized protocols for capturing and sharing SDOH data across various digital platforms. 
  • Data Analysis and Reporting: a. Analyze data from the CHW program to assess the impact of interventions, identify trends, and generate reports. b. Develop and implement data analysis tools for the CHW program to track progress, measure outcomes, and identify areas for improvement. c. Collaborate with IT to design and implement data analysis tools for assessing the impact of SDOH interventions, identifying trends, and generating reports. d. Ensure that data analytics systems align with the organization's strategic goals and objectives.
  • Training and Education: a. Collaborate with IT to develop and deliver training programs for staff on the effective use of digital tools, including the EPIC SDOH module and Unite Us platform. b. Provide ongoing support and troubleshooting assistance to ensure staff competency in utilizing digital resources.
  • Supervision of the SDOH Coordinator: a. Provide direct supervision of the SDOH Coordinator. b. Delegate tasks and responsibilities effectively. c. Conduct regular performance evaluations and provide feedback for professional development.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor's degree with +5 years data management experience or Master's degree in Public Health, or a related field with data management experience.
  • Prior experience working in data analytics and coordination, preferably within a large hospital system.
  • Familiarity with EPIC SDOH module and Unite Us or similar referral platforms. 
  • Experience with data analysis and reporting, preferably in a healthcare setting.
  • Strong communication, collaboration, and problem-solving skills.
  • Strong analytical and critical thinking skills.
  • Knowledge of community resources and social service agencies.
  • Experience supervising staff is preferred
  • Excellent written and verbal communication skills.
  • Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
