The Stroke Program Data Coordinator, for the Comprehensive Stroke Program is responsible for the coordination and facilitation of the stroke quality improvement program, which encompasses all disciplines and departments that participate in the care of the admitted stroke patient. This position ensures accurate data collection, analysis, interpretation and outcomes for improved patient care. She/he works under the direct supervision and direction of the Stroke Medical Director and to the Stroke Program Manager with indirect reporting to the Director of Network Neuroscience Quality.
Responsibilities
A day in the life of a at Hackensack Meridian Health includes:
- Responsible for monitoring, maintaining and enhancing the quality of care delivered to stroke patients using the Performance Improvement Process.
- Assists in data quality control and cleanup of abstracted indicators prior to submission to external sources
- Utilizes the Stroke Registry / Get with the Guidelines, Business Intelligence and Vizient to develop measures to provide a threshold for success and evaluation of processes.
- Utilizes expertise to build upon and enhance the electronic medical record to ensure compliance is met with best practice stroke science.
- Participates in real time data abstraction and analysis; validates data reports and collaborates with abstraction and coding teams
- Collaborates with network stroke leaders to harmonize best practices
- Identifies and implements data driven strategies to ensure success in patient transfers from referring network hospitals
- Covers day to day responsibilities of Stroke Program Manager and Stroke Program Coordinator at Primary Stroke Center in their absence (vacations, illness, position vacancy)
- Coordinates investigation, analysis and response to variances to core metrics defined by Joint Commission, quality/patient safety issues, events and significant care issues or trends. Works in collaboration with the Quality Department to assist in facilitation of root cause analysis, failure modes and effects analysis, and process improvement, working with multidisciplinary teams.
- Demonstrates use of appropriate methodologies and relevant tools such as PDCA, RCA, process flow and Affinity diagrams; assists team with understanding and interpreting their data. comprehensive stroke program
- Maintains a database for Joint Commission and prepares data presentation for comprehensive stroke certification
- Provides consultation and assistance to Primary Stroke Center spoke site to ensure quality data systems, performance improvement process and data analysis are in place
- Ensure that there is effective analysis of performance data with comparison over time and comparisons to internal and external benchmarks to identify improvement opportunities
- Documents performance improvement processes to determine success of actions and implementations and event resolution.
- In conjunction with clinical leaders, performs focused studies on diagnosis, procedures and various performance measures based on predetermined criteria. Participates in clinical research, presentation and IRB activities.
- Ensures plans and designs are consistent with internal and external expectations for certification, regulatory compliance and public reporting.
- Promotes the concept of cooperation and develops open lines of communication with department team members, patients, visitors, and other personnel within and outside the hospital.
- Works effectively with the Clinical Director, medical staff, neurosurgeons and vascular team to facilitate development and implementation of practice guidelines.
- Communicates problems or trends identified to appropriate committees, medical staff, administrative staff and/or management staff.
- Maintains current knowledge and expertise in regulations and requirements of a comprehensive stroke program. Ensures design and improvement plans are fully compliant with Joint Commission, American Heart Association, NJDOH, CMS, US News and World Reports.
- Adheres to HMH Organizational competencies and standards of behavior.
- Other duties and/or projects as assigned.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- Bachelor of Science.
- A minimum of 3-5 years of clinical hospital experience, preferably in neurology, intensive care, or emergency care.
- Strong presentation and facilitation skills.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Bachelor's Degree in Nursing.
Licenses and Certifications Required:
- NJ State Professional Registered Nurse License.
Licenses and Certifications Preferred:
- Certified Neuroscience Registered Nurse.
- Advanced Cardiac Life Support Certification.
- AHA Basic Health Care Life Support HCP Certification.
- National neuro certification within one year of hire (SCRN, CNRN, NVRN, etc).

Compensation
Minimum rate of $106,329.60 Annually
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

- Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
