The Social Work Associate works with residents by identifying their psychosocial, mental and emotional needs along with providing and/or aiding in the access of services to meet those needs. Services are provided with strict adherence to all government regulations.
The Social Work Associate is responsible for both fostering a climate and developing routines to enable residents to maximize their individuality, independence, and promoting the highest practical level of physical, mental, and psychosocial well-being and quality of life.
Responsibilities
A day in the life of a Social Worker at Hackensack Meridian Health includes:
1. Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychosocial needs.
2. For each resident, participate in the development of a written, interdisciplinary plan of care that identifies the psychosocial needs & issues, appropriate social work interventions, and the goals to accomplish. Document progress for each resident.
3. Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility.
4. Ensure or provide support and education to residents/family/significant others to assist in their understanding of the placement and facility issues in addition to referring them to appropriate social service agencies, when required.
5. Provide clinical interventions for residents to address catastrophic events that may occur.
6. Coordinate resident discharge planning process and make referrals for appropriate home care services before discharge.
7. Educate staff regarding the role of the Social Worker.
8. Educate staff regarding the psychosocial needs of residents and their families/significant others as well as the problems related to aging and disability.
9. Educate staff regarding cultural diversity as well as recognizing and preventing abuse, neglect, and mistreatment.
10. Educate staff, residents, and family/significant others regarding their rights and responsibilities; effective problem solving and the availability of community, health, and social services that are available, including post discharge support.
11. Develop and maintain a list of current community resources to assist residents and family/significant others.
12. Participate in the development and review of policies and practices related to Social Work as well as resident care & quality of life.
13. Understand and meet all government requirements, including documentation, certification, continuing education, and reporting.
14. Maintain confidentiality of all information.
15. Maintain full documentation and complete all administrative requirements, as required
Qualifications
Education, Knowledge, Skills and Abilities Required:
1. BA or Master's Degree in Social Work or a related field.
2. Knowledge of case work process, counseling and therapeutic process.
Education, Knowledge, Skills and Abilities Preferred:
1. Casework and group experience.
2. Long-term care facility experience
3. Prior work experience with geriatric and disabled populations.
4. Bi-lingual (Spanish)
Licenses and Certifications Preferred:
1. Certified Social Worker - Certification.

Compensation
Starting at $60,195.20 Annually
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate. - Labor Market Data: Compensation is benchmarked against market data to ensure competitiveness.
- Experience: Years of relevant work experience.
- Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
- Skills: Demonstrated proficiency in relevant skills and competencies.
- Geographic Location: Cost of living and market rates for the specific location.
- Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
