The Per Diem Post Acute Clinical Specialist reports to the Administrator, Oak Tree. This position is accountable for providing clinical support to support professional standards and care delivery as they directly relate to patient care, patient satisfaction and quality outcomes for post-acute care. In collaboration with the Administrator and SVP, Post Acute Ambulatory Care, provides consultative support and offers a strategic plan when needed for the care delivery system in post-acute. In addition, this position provides consultative support of the partnership with the third-party post-acute partnership vendor in relation to post-acute clinical services as needed.
Responsibilities
- Provides administrative post-acute clinical support ensuring post-acute care operations he/she supports according to established, approved procedures, quality control policies and in line with network nursing strategy.
- Collaborates with key stakeholders in HMH post acute site to develop and implement clinical and administrative objectives, policies, and procedures that are aligned with HMH's mission, vision and strategic objectives, and to ensure compliance with all regulatory agencies.
- Through audits and assessments, advises the facility to ensure operations are conducted in compliance with applicable accreditation, statutory and regulatory requirements, such as New Jersey (NJ) Department of Health (DOH), Centers for Medicare and Medicaid Services (CMS), any CMS deemed status accreditation agency, such as The Joint Commission (TJC), HIPAA, Corporate Compliance, and other department-specific regulatory agencies and accrediting bodies.
- Communicates audit results to Administrator and SVP Post Acute as appropriate and strives for excellence in patient care.
- Consults with key leadership and staff continually focusing on customer satisfaction and quality outcomes.
- Participates on committees as needed.
- Provides clinical consulting to HMH post-acute clinical operations at HMH owned entities.
- Supports the sites' compliance with federal, state, and local regulations pertaining to patient service centers and other entities of the post-acute facilities.
- Supports HMH with post acute consulting projects related to post acute care 3rd party quality performance.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- NJ State Professional Registered Nurse License.
- Bachelors (BA/BS/BSN) and Masters (MA/MS/MSN/MBA/MPA) degree are required. One degree must be in nursing.
- At least ten (10) years of progressive leadership in related fields is required.
- Ability to communicate effectively, both orally and in writing, and to react quickly and calmly in emergency situations.
- Strong strategic planning skills.
- Ability to establish and measure various metrics to assess success of the post-acute clinical operations and Vendor management agreement.
- Capacity to collaborate with diverse populations of team members and physicians to gain consensus and achieve results.
- Proven writing, public speaking and presentation skills.
- Excellent written and verbal communication skills.
- Proficient computer skills including but not limited to Microsoft Office and Google Suite platforms.
Compensation
Minimum rate of $53.81 Hourly

- Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
