The Patient Safety Specialist acts in conjunction with the HMH Patient Safety Officer and local Directors/Managers of Patient Safety & Quality to provide guidance for safety initiatives, including data analysis, identification of issues and recommendation. The Patient Safety Specialist collaborates with local safety leaders and HMH safety council members to achieve transformational and sustainable safety strategies using project/program management, process and system mapping, reliability science, data analysis, evidence based best practices and improvement strategies. These components and a partnership with frontline experts of clinical operations will provide structure and oversight to improve patient and team member safety. The identification, planning and execution of safety projects is necessary in order to deliver successful sustainable results and build a strong and sustainable culture of safety for patients and team members.

Prior clinical/ healthcare experience is strongly preferred for this position.

Responsibilities

A day in the life of a Patient Safety Specialist at Hackensack Meridian Health includes:

 

  • Facilitate highly functioning teams organized around safety initiatives and event driven improvement.
  • Engages and encourages a team members in the work of safety initiatives and improvement measures.
  • Review event reports as assigned and determine necessary follow up.
  • Bring safety and high reliability events, needs and concerns to weekly huddle or local huddle for follow up as necessary. Identify HMH priorities related to local safety committees.
  • Encourage event reporting and transparency related to high reliability events, concerns, needs, processes to identify precursor and safety events.
  • Develop & implement data driven strategies to address the identified priorities.
  • Access effectiveness of implemented strategies using specific metrics designed to uncover gaps or other failures in risk reduction activities.
  • Ensure continuous learning through close the loop communication locally and through HMH Safety Council, Safety Event Classification call and updating the DPSSQ safety page on the intranet.
  • Provide knowledge, guidance and mentoring in patient safety initiatives utilizing safety and high reliability science.
  • Provide coaching and hands-on support of safety strategies.
  • Increase transparency, reinforce high reliability and advance the safety culture by participating in and facilitating safety huddles, patient safety activities, Just culture reinforcement and shared learning.
  • Participate in the NPSF RCA-2 process across HMH. Provide, guidance, support and direction resulting in shared learning, across the institution and HMH.
  • Advance the science of safety and quality improvement through dissemination of methods, results and lessons learned.
  • Present safety strategies, successes, methods and results at local, national and international forums.
  • Implement trend analysis of safety events with comparison over time to internal and external benchmarks.
  • Advance & enhance professional growth and development through seminars, workshops and professional networking opportunities to maintain and extend expertise.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • 5 years of increasing responsibility related to data analysis, presentation and desire to participate in patient safety work.
  • Bachelor's Degree.
  • Proficiency in the use of computer applications and software including Google applications including Sheets, Docs and Slides
  • Excellent written and oral communication skills with ability to present to various levels within the organization.
  • CPPS within one year of hire.

 

Education, Knowledge, Skills and Abilities Preferred:

  • Performance Improvement experience.
  • Experience in statistics or research.
  • Understanding of The Joint Commission and Regulatory Standards.
  • Master's degree.

Licenses and Certifications Required:

  • Certified Professional in Patient Safety within 1 years of entering position.
  • Open School Patient Safety Basic Safety Certification within 6 months of entering position.

Licenses and Certifications Preferred:

  • Certified Professional in Patient Safety.
  • Open School Patient Safety Basic Safety Certification.

Compensation
Starting at $82,513.60 Annually

HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate.   • Experience: Years of relevant work experience.
  • Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
  • Skills: Demonstrated proficiency in relevant skills and competencies.
  • Geographic Location: Cost of living and market rates for the specific location.
  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.

In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
