The Psychosocial Assistant provides case management services to Oncology patients at The Cancer Center with the goal of increasing the access to concrete services. Patients include adults 16-64 years old and geriatrics 65+ years.
Responsibilities
A day in the life of a Psychosocial Assistant at Hackensack Meridian Health includes:
- Implements case management for medical and psychology needs of the oncology patient.
- Works in collaboration with doctors and nursing staff for pharmaceutical assistance for all Cancer Center patients.
- Notifies patients without prescription coverage of pharmaceutical assistance programs.
- Assists needy patients to complete patient assistance application.
- Refers eligible patients to state pharmaceutical programs
- Refers to nonprofit organizations providing pharmaceutical assistance
- Screens and approves distribution of funds from Foundation programs or other financial assistance programs for emergency medications or living expenses as available.
- Psychosocial Support (under supervision) for patients/families experiencing: New diagnosis, patients/family having difficulty following their treatment plan, and patients/family dealing with end of life issues.
- Identifies patient's emotional needs and refers to Social Worker.
- Participates in professional and patient educational programs offered by Social Work Department.
- Assists with discharge planning as directed by social worker when needed.
- Orients patients to their local Board of Social Services, Office on Aging, etc. to obtain eligibility for various state programs. (i.e. Medicaid, NJ Protect, Food Stamps, etc.)
- Orients patients to their local Social Security office for long term disability.
- Refers questions/concerns regarding short and long term disability to Social Worker
- Refers and applies to various cancer foundations for monetary assistance. (i.e. Cancer Care, LLS)
- Refers patients to support groups and advocate groups in their local area and in their primary language
- Liaison for The Cancer Center and Charity Care.
- Assesses and navigates patients needing transportation resources to local agencies. (i.e. Logisticare, Access Link, emergency transportation as available)
- Other relevant duties as designated by Supervisor.
- Adheres to the standards identified in the Medical Center's Organizational Competencies.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- Bachelor's degree from an accredited college or university or equivalent work experience.
- Experience in handling multiple tasks.
- Computer proficiency (Microsoft Office, etc.)
Education, Knowledge, Skills and Abilities Preferred:
- Bachelor of Social Work

Compensation
Starting at $22.77 Hourly

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
