Responsibilities

The LPN, Face to Face Liaison obtain, track and monitor face to face documentation to ensure compliance with Medicare regulations. Act as liaison between Home Care services and Intake Department. Review clinical documentation from physician offices, hospitals and facilities as it relates to face to face encounters.

 

  • Collect all face to face documentation for all Medicare referrals. Responsible for ongoing compilation from Soarian, Sigma Care, physician office and any other systems deemed necessary. Document in appropriate system.
  • Monitor and obtain face to face documentation from physicians according to regulatory guidelines for patients admitted to home care services.
  • Communicate with all Intake staff members to ensure all face to face documentation is collected timely, accurately and documented according to the established process.
  • Responsible for running daily face to face system reports and any other reports as needed to ensure accurate collection of outstanding face to face documentation. Update and maintain face to face log.
  • Collaborate with operations face to face liaisons through daily open communication in order to facilitate an increased completion rate.
  • Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities at all times.
  • Adheres to HMH's Organizational competencies and standards of behavior.
  • Lifts a minimum of 10 lbs., pushes and pulls a minimum of 10 lbs. and stands a minimum of 4 hours a day.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Graduate of LPN Program
  • Strong Customer Service Skills
  • Excellent written and verbal communication skills
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms

 

Education, Knowledge, Skills and Abilities Preferred:

  • Home Care Experience

 

Licenses and Certifications Required:

  • Licensed Practical Nurse
  • BLS/CPR certification by American Heart Association is required within 60 days of hire

 

Compensation
Minimum rate of $28.93 Hourly

  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
