Responsibilities
The Licensed Practical Nurse in the Family Health Center is an integral part of the health care team. The LPN will provide nursing support to the professional team by providing direct patient care in terms of managing a high volume of patients coming into the Family Health Center for a variety of appointment types. The LPN will interview and room patients, ensuring all regulatory requirements are maintained. The LPN will provide follow up for various clinical sessions, and will maintain care in the appropriate documentation format, meeting all Joint Commission and patient safety requirements. Works closely with physicians to ensure plan of care is complete and the patient understands the plan. Provides instruction and education to patients and families. Coordinates medication refills, phone calls, appointments, and surgical scheduling. Will work in women's health, gynecology, and adult medicine. Must have experience working in pediatrics and can confidently administer pediatric vaccines. Must provide care in an organized and patient centered manner. The LPN will maintain all required nursing competencies on a yearly basis. Able to act in accordance with standards in case of patient emergency. Must be able to communicate in a positive manner with staff, patients and physicians. Must have a basic understanding of insurance plans and implications on the path for care
- Administration of medications and vaccines, including pediatric vaccines
- Scheduling appointments
- Rooming of patients including vital signs, nursing intake, updating health summary
- Documents in the medical record in accordance to standards
- Manages a large volume of phone calls, coordinates prescription refills
- Filing, management of the integrity of the medical record
- Coordinates care between primary and specialty care and assists the patient with navigation of the health care system
- Other duties as assigned including assistance with venipuncture and working in all clinical areas of the Family Health Center
- Scheduling of procedures, tests, surgeries if needed
- Performs other duties as needed such as completing referrals and coordinating prior authorizations
Qualifications
- Current and valid New Jersey Practical Nurse license required
- Experience administering adult and pediatric vaccines required
- One year experience in OB/GYN procedures preferred
- One year of outpatient pediatric nursing experience preferred
- Up to date education credentials in the Vaccine for Children program requirements preferred
