The position is responsible for the daily coordination of care for medical travel patients. This position will work with appropriate leaders and team members, including the lead Advanced Nurse Practitioner and Neuroscience Operations Manager for neuroscience patients, to ensure an optimal patient experience. The job responsibilities include but are not limited to navigating the patient through the care process and being the liaison between our providers and the hospital.

Responsibilities

A day in the life of a Global Healthcare Coordinator at Hackensack Meridian Health includes:

 

1. Handles coordination between provider offices and the hospital for medical travel patients.
2. Collaborates with surgical services to communicate information to and from provider offices to support care coordination.  Communicates information to patients at initial and follow up phone calls. Documents interactions.
3. Demonstrates clear contributions to reaching the business volume and revenue goals on a monthly basis by efforts including implementing creative solutions to overcome challenges and supporting the medical travel program.
4. Develops or revises forms and tools necessary to promote the efficient operations of the department.
5. Utilizes IT whenever possible to streamline the efficiency of operations and makes recommendations to modify current forms or documents to better meet the department's needs.
6. Responsible for maintaining reports required by Global Healthcare Accreditation to be in compliance with the GHA accreditation.
7. Helps to coordinate care and accommodations for medical travel patients.
8. Supports community and global outreach planning.
9. Provides excellent customer service. Helps promote high levels of service, greets patients and visitors in a prompt, courteous and helpful manner. Responds promptly to patients and listens with empathy both in person and on the phone.
10. Manages time efficiently to meet the needs of the department. Performs assignments with efficiency and accuracy in a positive manner.
11. Other duties and/or projects as assigned.
12. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

1. High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
2. Minimum of 1 or more years of experience in healthcare.
3. Effective interpersonal and communication skills.
4. Excellent written and verbal communication skills.
5. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

 

Education, Knowledge, Skills and Abilities Preferred:

1. Bachelor's Degree.
2 Two or more years of experience in healthcare.
3. Proficient in medical terminology and HIPAA guidelines.
4. Knowledgeable in the field of medical travel.
5. Familiar with the hospital's location and community resources.
6. Bilingual.
