The Community Health Worker (CHW) provides social support, navigation, and advocacy to diverse patient populations, including high-risk adults, children, and their families. The CHW acts as a trusted community liaison to help patients address the social, medical, and community challenges that can lead to poor health. The goal is to help patients and their families manage real-life issues that impact their well-being, assist with navigating the healthcare system, and work towards health goals by managing chronic conditions, adhering to care plans, and promoting healthy behaviors.
Responsibilities
A day in the life of a Community Health Worker at Hackensack Meridian Health includes:
- Initiate contact with eligible patients and their families telephonically or in-person in clinical, community, or home settings.
- Conduct social needs screenings and assessments to identify barriers to care, and assist patients in setting health goals.
- Provide emotional, informational, and instructional support to patients and families.
- Conduct weekly follow-ups with patients via their preferred method of communication (e.g., telephonic, home, or community visits).
- Advocate for patients by motivating them to meet health-related goals and educating them on how to navigate complex systems.
- Connect patients and families to community-based resources to address social determinants of health, such as food insecurity, housing instability, medication needs, and transportation needs.
- Assist patients with completing applications for benefits and community programs, organizing paperwork, and scheduling appointments.
- Serve as a liaison and collaborate with patients, clinical care teams (e.g., clinicians, care managers) and community-based organization stakeholders to coordinate care and address patient needs.
- Provide patient education on health conditions, treatment adherence, and preventative care, tailored to the patient's specific needs and health literacy level.
- Promote patient's and patient support's adherence to medical treatment through assessment of readiness to make changes through identification of barriers and development of mutually agreed upon compliance strategies.
- Identify and re-engage patients who are lost to care.
- Document all patient encounters, screenings, and interventions accurately and in a timely manner in the Electronic Medical Record (Epic) or other required systems.
- Work effectively in a hybrid model, including office, clinical, community, and home-based settings as required.
- Work collaboratively with other CHWs and staff to create and maintain a directory of community resources.
- Attend and participate in regular team meetings, one-on-one meetings, and required trainings.
- Prepare reports and documents as needed or requested.
- Attend community events to promote health and wellness of various patient populations.
- Adheres to HMH Organizational competencies and standards of behavior.
- Other duties and/or projects as assigned.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- Bachelor's degree in health related field or a High School diploma, general equivalency diploma (GED), and/or GED equivalent programs with 3+ years experience in health or hospitality field
- Vast Knowledge of social services resources available to respective communities.
- Ability to exercise judgment in the application of professional services.
- Comfortable with home visits and outreach.
- Willing and able to travel for home visits.
- Excellent written and verbal communication skills.
- Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Prior experience as a Community Health Worker, Outreach worker, or similar role.
- Prior experience working with diverse or high-risk populations such as pediatric patients or patients with chronic diseases
- Bilingual/multilingual with ability to pass a language fluency test
- Long-term resident of the service area with vast knowledge of local social service resources.
Licenses and Certifications Required:
- BLS AHA Basic Health Care Life Support HCP Certification.

Compensation
Starting at $22.78 Hourly
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.
The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate. - Labor Market Data: Compensation is benchmarked against market data to ensure competitiveness.
- Experience: Years of relevant work experience.
- Education and Certifications: Level of education attained, including specialized certifications, credentials, completed apprenticeship programs or advanced training.
- Skills: Demonstrated proficiency in relevant skills and competencies.
- Geographic Location: Cost of living and market rates for the specific location.
- Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
In addition to our compensation for full-time and part-time (20+ hours/week) job positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
