Responsibilities
Responsible for all aspects of patient scheduling for Hackensack Meridian Health At Home Infusion.
- Acts a liaison between referral source, intake department, pharmacy, warehouse/delivery courier and nursing department to coordinate the home discharge/Infusion start of care. Ensures the timely coordination of patient drug and supply delivery to coincide with patient's therapy-specific visit date/time requirement.
- Coordinates, as necessary, to contract supplemental staffing agency for per-diem visits for Start of Care or Routine visits. Ensures that supplemental staffing agency is provided with a copy of the prescriber's order, patient's medication list and patient's plan of care.
- Reconciles assigned supplemental staffing agency visits to ensure timely receipt of all visit-related documentation within 48 hours. Scans received documentation into the EMR system within 48 hours & enters transcription contact into the EMR system.
- Acts as liaison between pharmacy department and Medicare Part-A certified nursing agency to coordinate timely clinical care (ie. Venipuncture, testing, etc.) for `product-only` Home Infusion cases.
- Responsible for managing the laboratory testing requests. Ensures that all prescriber-ordered laboratory specimen results are received from the testing laboratories in a timely manner. Ensures laboratory results are forwarded to the prescribing provider, pharmacist and Manager of Clinical Practice for review and appropriate disposition.
- Schedules patient subsequent nursing visits based on prescriber's orders. Notifies supervisor when requests are received that require a new/updated prescriber's order.
- Verifies nursing visit authorizations and notify the billing department when additional units/authorization is required.
- Tracks the timely completion of patient consents and emergency plans by department staff and scans completed documents into EMR. Notifies the Manager of Clinical Practice of any incomplete or inaccurate documents that require corrections by department staff.
- Tracks interim orders and plan of care documents through to signature completion in compliance with regulatory guidelines. Responsible for ongoing follow-up, faxing or electronic delivery of signature requests. Notifies Manager of Clinical Practice of unsigned orders that are at risk of non-compliance at least 7 days before the due date for appropriate escalation.
- Validates copies of nursing documentation against schedules and reconciles incongruities. Notifies supervisor of outstanding documentation and interface with payroll and billing processes. Verifies all visits accurately and validates in the EMR scheduling module for payroll.
- Recognizes patient grievances and directs them to the appropriate manager.
- Assists On-call nurse with weekend and/or daily on-call process. Schedules weekend Routine and Start of Care visits in a timely manner and enters into the EMR scheduling system.
- Answers telephone promptly and in a courteous manner. Takes accurate and complete messages and relays them to the appropriate party. Provides telephone triage and dispositions urgent concerns to the appropriate clinical staff to disposition/resolve if outside of the scope of Infusion Nursing Coordinator.
- Maintains appropriate stock of nursing supplies, laboratory specimen supplies and departmental documents, training materials and miscellaneous forms. Orders replenishment of supplies and forms in a timely manner to ensure minimum PAR levels are maintained.
- Faxes all transfer forms to appropriate facilities and coordinators within 24 hours.
- Other duties and/or projects as assigned. Complete other duties as assigned and demonstrate flexibility with job responsibilities in all areas.
- Adheres to HMH`s Organizational competencies and standards of behavior.
- Lifts a minimum of 10 lbs., pushes and pulls a minimum of 10 lbs. and stands a minimum of 2 hours a day.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
- Minimum of two (2) years of Customer Service experience.
- Proficient in Microsoft Office Suite / Google Productivity Suite.
- Strong Organizational Skills.
- Ability to function autonomously.
- Must be able to multi-task in a high-volume, high-paced environment.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Healthcare Experience.
- Knowledge of Medical Terminology.
- Associate degree or higher.
Compensation
Minimum rate of $23.13 Hourly
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

- Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.
Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
