The Supervisor, Case Manager, East Mountain Youth Lodge (EMYL) at Carrier Clinic provides administrative case management support to EMYL's clinical and leadership teams in maintaining proper clinical documentation, signed consents, and psychoeducational plans for EMYL residents.

This Supervisor will provide administrative case management support to help ensure compliance with the New Jersey Department of  Children and Families (DCF), Division of Children's System of Care (CSOC) licensing standards for Intensive Residential Treatment Services (IRTS), Residential Treatment Center & Behavioral Health/Substance Use (RTC & RTC/BH/SU), and Psychiatric Community Home (PCH) programs at  EMYL. Additionally, they will supervise all Case Managers and work closely with and report to the Director of Administrative Operations.

Responsibilities

A day in the life of a Supervisor, Case Manager at Hackensack Meridian Health includes:

 

1. Plan treatment plan meetings and all subsequent meetings with DYFS and involve other relevant stakeholders. Set up schedules to keep in compliance with all licensing requirements for treatment planning.
2. Provide liaison services to all involved professionals and non-professionals under the direction of the Director of Admin Operations, EMYL.
3. Oversee and ensure the completion of all required documentation, notifications, and consents for residents' charts.
4. Work closely with the East Mountain Youth Lodge Nurse Manager, Clinical Manager, Residential Manager of Quality and Compliance, and staff to ensure compliance with all licensing regulations.
5. Supervise the case managers for completion of their duties. Completes yearly performance reviews and meets for 1:1s with their direct reports on a weekly basis.
6. Attend daily safety huddles for EMYL.
7. Be on call in rotation to respond to urgent staff needs or shortages.
8. Be available to attend or run emergency meetings with staff and residents.
9. Oversee and ensure that all medical appointments and psychological evaluations are made within the required time frames.
10. Make sure all youth residents have an education plan in place.
11. Attends leadership meetings for each program.
12. Conducts orientation groups for parents as needed. Works with the Staffing Coordinator to ensure team members complete annual training.
13. Attends meetings with the Adolescent unit staff when residents are hospitalized.
14. Review all medical charts for compliance with all regulatory agency guidelines.  Communicates any deficiencies to the appropriate staff and follows up to ensure compliance. Collaborates with the Director of Admin Ops and Quality Manager to remedy identified chart compliance issues.
15. Other duties and/or projects as assigned.
16. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

1. Bachelor's degree in a relevant field with a minimum of 5 years of relevant experience or a Master's degree with 3 years of relevant experience.
2. Excellent written and verbal communication skills.
3. Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.

Compensation
Minimum rate of $68,473.60 Annually

  • Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization.
  • Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
