Bridging the Gap – Graduate Nurse Practitioner Academia to Practice
Our mission is to bridge NP academia to practice by providing additional education, clinical, and classroom experience. Together, we foster a supportive learning environment for academic, professional, and personal success.
Hackensack Meridian Health (HMH) is proud to be one of the first Primary Care Nurse Practitioner Residency/Fellowship Programs in New Jersey. We are honored to be chosen as one of 36 programs in the United States to receive a grant from Health Resources and Service Administration (HRSA) Advanced Nursing Education (ANE). The purpose of the HRSA-ANE Grant is to increase the number of Advanced Practice Nurse primary care providers in community-based settings.
In partnership with our academic partner, Monmouth University, HMH has created a rigorous nurse-practitioner residency program in community based primary care practices in NJ Counties of Atlantic, Monmouth, Ocean, Middlesex and Bergen.
This program is a one-year, full time commitment designed to provide the new NP Clinician with a unique educational opportunity to focus on clinical experience in outpatient primary care, specialty medical practices, psychiatry and urgent care centers specific to the Nurse Practitioner’s Population Foci. Didactic sessions will supplement the clinical experience with Grand Round Presentations, quality improvement projects, journal club, SIM skills lab and lectures to provide both personal and professional growth.
Interested candidates who meet all the criteria including the graduation criteria (graduated on or after January 1, 2021), are eligible to apply. Applicants should have a strong desire to work in primary care/outpatient settings, FQHCs or other settings that serve the uninsured/underinsured/underserved populations at the completion of this program and meet the other stated program criteria.

Description
Program Educational Goals
A postgraduate residency program provides a bridge from formal education to successful practice. Our program will build upon your foundation of clinical knowledge, experience, and critical thinking and aid to further develop skills necessary for practice. It is also designed to assist the new practitioner to understand more about the administration, financial, and operational aspects of practice.
In partnership with our academic partner, Monmouth University, HMH has created a rigorous nurse-practitioner residency program in community based primary care practices in NJ counties of Atlantic, Monmouth, Ocean, Middlesex and Bergen.
This is a grant program awarded to HMH by HRSA with the expectation that our NPs will consider working in Health Centers that care for the underserved. Clinically, the focus is to address primary care/family practice, mental health issues, addressing pain and opioids, telehealth as well as completing QI/QA projects that identify real life opportunities to improve and transform care.
Goals:
- Our program will increase access to quality primary care for underserved and special populations by training our Nurse Practitioners in a FQHC-based residency program that prepares residents for full and autonomous expert care of complex underserved populations across all life cycles and in multiple settings.
- Provide new Nurse Practitioners with a depth, breadth, volume and intensity of clinical training necessary to serve as primary care providers.
- Increase the number of Nurse Practitioners ready to serve in leadership roles and understand more about the administration, financial, and operational aspects of practice in community health settings.
- Increase the number of Nurse Practitioners ready to serve in leadership roles and understand more about the importance of legislation, regulatory law, advocacy and professional practice.
Program Structure
- This is a paid 12-month residency.
- Clinical rotations are supplemented with didactic sessions, Grand Rounds, Journal Club and other learning experiences.
- Didactic sessions weekly with expert content providers provided through Hackensack Meridian Health as well as Monmouth University School of Nursing.
- Assigned Clinical Rotations in primary care, behavior/ mental health, urgent care or pediatric sites. Residents are assigned a mentor to act as a resource and provide support as they transition to autonomous nurse practitioners.
- Applicants should have a desire to work in primary care, mental health, FQHCs or other safety settings within underserved communities.
- Upon successful completion of the residency, a certificate of completion is awarded.

A comprehensive benefit package that includes paid vacation, sick leave, holidays and health benefits.

Grace Reilly Mazzatta, MSN, RN, APN-C, ANP-BC is the Program Director for the Health Resources and Service Administration (HRSA) Advanced Nursing Education (ANE) Graduate Nurse Practitioner Residency Program for the Ann May Center at Hackensack Meridian Health University Medical Center. Grace is board certified as an Adult Nurse Practitioner since 2000 with experience in Palliative Care, Family Health Center/Charity Care Clinics, Cardiology, GI, Dialysis and Critical Care. She received her BSN from Rutgers University and her MSN from the University of Medicine and Dentistry. She currently is working in the Anne May Center for Nursing as the Program Director for the HMH, HRSA ANE-NPR Grant to establish a graduate Nurse Practitioner Residency Program. Grace has leadership experience in the NJ State Nurses Association, the Forum of Nurses in Advanced Practice, the American Association of Nurse Practitioners, and APN-NJ. Grace is committed to and passionate about her profession as an Advanced Practice Nurse, and has participated in legislation and regulatory affairs, including testifying and lobbying to pass legislation to allow APNs to sign Death Certificates and has worked on the Consumer Access to Health Care Bill.

Qualifications
- U.S. Citizen
- Graduate of Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing (ACEN) accredited nurse practitioner programs.
- Recent graduate within 18 months of start date.
- Agree to full time participation in HMH GNPRP for 12 months.
- Hold a master’s degree or Doctorate in Nursing and be licensed as a nurse practitioner in New Jersey, nationally board certified through the ANCC or ACNP with a specialty in Adult/Geriatric, Family or Psychiatric-Mental Health with focus on primary care.
- Proficient in written and spoken English, bilingual preferred.
