Responsibilities

The Nurse Practitioner is responsible for the provision of high-quality nursing care that is compliant with federal, state, and local regulations. They are responsible for rendering professional nursing care to patients in their home by assessing, developing, implementing and evaluating assigned patients in keeping with organization policies and procedures, external regulations, and the Advanced Practice Act based on the CMS Acute Hospital Care at Home waiver model of care. The Hospital at Home program is designed to provide inpatient level of care in the home setting in collaboration with the hospital physician, select hospital departments and community providers.

 

  • Clinical decisions are made utilizing professional standards of care and code of ethics and provide for safe and effective patient outcomes. Document clinical decisions in an appropriate manner.
  • Display expert, complex, clinical decision-making skills, triage and collaboration skills, and will display a high-level of comfort with providing care in a non-traditional setting.
  • Assure assessment, planning, intervention and evaluation of patient care are in accordance with policy and procedure, Advanced Practice Act and Medicare guidelines. Utilize a holistic manner in caring for patients/significant others. Assure safe patient care is provided.
  • Implement specific teaching strategies to meet goals established on the patient plan of care. Assure appropriate teaching strategies, consistent with goals and plans, are documented in the medical chart.
  • Assure documentation of assessment, interventions and planning meet the needs of the patient, family and all regulatory bodies.
  • Communicate pertinent patient information to physicians and Hospital at Home care team (where applicable). Act as liaison among physicians and other members of the hospital at home care team to ensure effective coordination of care.
  • Maintain awareness of new/updated standards and incorporate them into his/her clinical practice throughout the continuum of care. Demonstrate knowledge of current practices guidelines in documentation in the medical chart.
  • Provide on-call coverage for designated shifts when required. Maintain phone contact with offices as appropriate. Respond to calls within designated time frames and physician orders.
  • Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.
  • Adheres to HMH's Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Master's Degree in Nursing.
  • Minimum of five (5) years of experience as a Registered Nurse.
  • Minimum of two (2) years of clinical experience as a Nurse Practitioner.
  • Ability to learn quickly and adapt to changing patient needs, a strong sense of accountability for improving the lives of our patients and their caregivers, an exceptional focus on teamwork, dedication to ongoing education and the ability and passion to deliver the highest quality of care based on a strong sense of patient focus.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
  • Physical Demands: - Long periods of sitting with frequent standing, walking, or moving. - Requires periods of keying on the computer terminal to input information. - Requires constant verbal communication in person or using the phone. - Requires frequent reading of correspondence, emails, reports, materials, etc. - Exposure to communicable diseases. - Lifting >35 pounds. - Extended wearing of PPE. - Application of surgical mask for long periods of time.

Education, Knowledge, Skills and Abilities Preferred: Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.
  • Advance Practice Nurse License.
  • AHA Basic Health Care Life Support HCP Certification.
