The Social Worker LCSW delivers Mental Health Services to patients, including the assessment and ongoing treatment of Mental Health conditions.


A day in the life of a Social Worker at Hackensack Meridian Health includes:

  • Function as a treatment team member at the off-site Counseling Center to coordinate and integrate treatment planning of patients. Deliver services through individual, family, or group scheduled psychotherapy/counseling sessions. Utilize knowledge of techniques and regulations appropriate to the ages and conditions of patients served. Communicate clearly and respectfully with patients and respond promptly to patient inquiries and requests.
  • Assess patients through telephone calls and/or in-person intake sessions for treatment needs and appropriateness for services at PMC. Refer non-appropriate persons for services through coordination with other providers for mental health and health needs. 
  • Document all patient contacts, and relevant patient material, through the Epic electronic health record in a timely fashion. Maintain and update all records regularly, including ongoing treatment planning. Maintain, and update, all skills related to the electronic health record and any changes occurring within it. 
  • Manage ongoing caseload effectively to determine frequency and duration of patient contact, including planned, and unplanned, terminations. Engage and educate patients about responsibilities including attendance expectations. Maintain a 70% overall productivity rate for days worked. 
  • Utilize weekly clinical supervision productively through engagement in the process and through self-evaluation and discussion of growth areas. Concurrently, participate actively in weekly treatment team meetings, and monthly peer supervision, to problem solve clinical and systemic issues facing patients and staff. 
  • Identify and evaluate risk-issues facing patients, and act upon them through treatment team meetings, supervision, consultation with the agency psychiatrist, or emergency based services as needed. Provide safe and effective treatment for patients who should be included at the Counseling Center and make timely referrals for persons who are beyond the agency’s scope of care.
  • Ensure patient safety and privacy at all times, including, but not limited to, observing all regulations of HIPAA and regulations for mandatory reporting to the New Jersey Department of Child Protection and Permanency (DCPP) and the New Jersey Adult Protective Services.
  • Participate in Continual Quality Improvement efforts through participation on departmental and interdepartmental CQI teams
  • Maintain LCSW Social Work licensure in good standing in the State of New Jersey including obtaining mandatory annual continuing education, with documentation of satisfactory attendance.
  • Provide clinical supervision of social work graduate interns, as available. 
  • Update knowledge of regulations and techniques appropriate to ages of patients served by participating in staff training and educational programs.  
  • Other duties and/or projects as assigned.



Education, Knowledge, Skills and Abilities Required:


  • Masters degree in Social Work
  • Minimum 2 years of post-graduate experience in out-patient mental health service delivery
  • Excellent written and verbal communication skills.
  • Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  • Bilingual in English and Spanish preferred

Licenses and Certifications Required:

  • NJ Social Work license LCSW required.
