Assist the Director of Nursing in providing direction and supervision to nursing staff to ensure quality resident care is given on assigned unit. Ensures all federal/state regulations for documentation and resident care are met. Monitors staff to ensure all facility policies and procedures are followed. Acts as a liaison between resident, families, other departments, and outside services to meet resident needs.


1. Assesses and evaluates staff to ensure they are providing quality care.
2. Ensures staff are knowledgeable of resident care and equipment use.
3. Orients new employees to the unit policies and procedures.
4. Works with Interdisciplinary Team to ensure quality services are provided.
5. Ensures all required documentation is completed as per state/federal guidelines.
6. Participates in facility Performance Improvement Program.
7. Attends all required meetings as per Director of Nursing.
8. Participates in hiring process as needed.
9. Disciplines staff as needed.
10. Holds monthly communication staff meetings for all shifts and care conferences with patients, families, and the Interdisciplinary Care Team.
11. Accountable for 24-hour resident care provided on assigned unit.
12. Assists Director of Nursing as needed.


Education, Knowledge, Skills and Abilities Required:

1. Completion of an approved LPN program.
2. Current with all Continuing Education (CE) requirements.
3. Perform clinical care professionally.
4. Serve as patient advocate and role model to other Team Members.
5. Deliver patient care with minimal disruptions and obstacles.
6. Ability to effectively work on a multidisciplinary team.
7. Computer skills, including Microsoft Office.

Education, Knowledge, Skills and Abilities Preferred:

1. Long-term care facility experience.
2. Prior work experience with geriatric and disabled populations.

Licenses and Certifications Required:

1. Licensed Practical Nurse.
2. Basic Cardiac Life Support Certificate.
3. Cardiopulmonary Resuscitation Certification.
4. IV Certification.
