Responsibilities

 

  • Facilitate the professional management of the intake process through coaching and mentoring the intake team. 
  • Establish and maintain ongoing relationships with referral sources. 
  • Develop and maintain current home care services that can be provided in the marketplace.
  • Implement an effective referral process.  Facilitate and effective referral process that is both timely and complete.  Develop tools and standards to assure an efficient flow of information.
  • Ensure patient appropriateness for consistency with home health admission criteria, Medicare and other payer specifications.
  • Assure performance improvement of the department.  Oversee quality and accuracy of all entries into appropriate systems.  Perform ongoing audits and address inaccuracies using audit tools.
  • Measure and maintain productivity standards with expense control measures in place.
  • Establish and maintain relationships with key individuals in internal and external facilities.  Develop and open communication system with referral sources, surfacing issues and developing business initiatives.
  • Complete a thorough review of reimbursement issues for all referrals in order to maximize home care reimbursement.  Resolve all billing issues that surface

Qualifications

Knowledge, Education and Skills Required:

 

  1. RN or PT License
  2. Five (5) Years Healthcare Experience
  3. Two (2) to Three (3) Years Management Experience
  4. Proficient in Microsoft Office Suite
  5. Referral Database Experience
