Responsibilities

  • Complete patient evaluation 
  • Develop an individualized treatment program procedure. 
  • Assure documentation is precise, neat and legible. 
  • Assure that service delivery follows the frequency an duration on the physician’s plan of care for the client.
  • Educate the patient and/or caregiver in a written home program.  
  • Provide comprehensive education to patients and/or caregivers based upon their identified needs in a manner that they are able to understand.

 

Qualifications

  • Knowledge, Education and Skills Required: 

    • Bachelor’s Degree in Occupational Therapy
    • Minimum of One (1) Year OT Experience
    • PC Experience/Skills

     

    Knowledge, Education and Skills Preferred: 

    • Master’s Degree
    • Home Care Experience

     

    Required Certification/Licensing:   

    • Registered Occupational Therapy License
    • CPR Certification
    • BLS
