The Clinical Coordinator, Wound Care communicates and collaborates with physicians and other members of the health team to provide consistent quality care for patients with wounds. S/He assists patients with wound healing modalities to achieve the level of independent functioning possible. S/He complies with the hospital and nursing philosophy and standards while performing job duties and responsibilities. S/He will be responsible for knowledge of, adherence to, monitoring of and compliance with all regulatory and licensing agency guidelines including but not limited to TJC, OSHA, AHCA, CMS, etc.

Responsibilities

  1. Communicates and coordinates with physicians and other members of the multi-disciplinary team regarding clinical decisions and best practices to produce desirable outcomes.
  2. Assesses, plans, implements and evaluates plans of care.
  3. Supports surgeon and primary care provider needs for testing, prescription refills, patient instructions and documenting/tracking findings.
  4. Provides formal and/or informal patient education.
  5. Acts as a resource for clinical staff and develops guidelines/protocols used in planning patient care and monitors outcomes.
  6. Improves processes to improve patient outcomes.
  7. Other duties and/or projects as assigned.es
  8. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

  1. Graduate of an approved school of professional nursing.
  2. Minimum of 2 years’ experience as a professional Registered Nurse- RN.
  3. Bachelor of Science in Nursing required within 3 years of hire
  4. Minimum of 2 years’ experience as a Wound Care RN in an acute care setting preferred
  5. NJ State Professional Registered Nurse License.
  6. BLS- AHA Basic Health Care Life Support HCP Certification.
  7. National Certification in Wound Care within 6 months of hire.
