The Transplant Waitlist Navigator facilitates the coordination of care of waitlisted solid organ transplant candidates to maintain status on the transplant waitlist and ensures program regulatory compliance with the Centers for Medicare and Medicaid Services (CMS) and theUnited Network for Organ Sharing (UNOS). Ensures required annual and as needed candidate testing is up to date per CMS, UNOS and program guidelines.

Responsibilities

A day in the life of a Transplant Waitlist Navigator at Hackensack Meridian Health includes:

 

  • Promotes the facility mission, vision and values by effectively communicating them to others.
  • Considers mission, vision and values in developing services, standards and practices.
  • Considers priorities and special needs of the patient, physician, and the organization or clinic when scheduling appointments and procedures.
  • Ensures the facilitation of the clinical aspects of care for transplant program waitlisted patients
  • Collaborates with Physicians and Mid-Level Providers with scheduling waitlisted patients for all required testing, studies, appointments required to remain on transplant waitlist
  • Collaborates with Physicians and Mid-Level Providers to maintain current medical and psychosocial clearances in accordance with regulatory and program guidelines.
  • Acts as liaison between the transplant program, waitlisted candidates, and dialysis centers
  • Acts as liaison between transplant program and tissue typing lab to maintain current sera samples for all waitlisted candidates
  • Maintains readiness of the top of the waitlist for each blood group
  • Requests testing, procedures, re-evaluation appointments according to policies, nursing and department staff availability, patient needs, and physician¿s block time.
  • Provides education to patients and their families in collaboration with Physicians and Mid-Level Providers regarding their waitlist status, required testing and procedures, follow-up plans of care, and program requirements in order to maintain status on transplant waitlist.
  • Runs, compiles, and analyzes waitlist and waitlist patient data as needed for administrative use.
  • Prepares the re-evaluation clinic schedule for timely distribution.
  • Send out provider letters, candidacy letters, and patient communication letters as required by CMS.
  • Completes OPTN reporting as required
  • Participates in the Multidisciplinary Selection Committee and presents waitlisted patient cases for activation, inactivation, or de-listing.
  • Coordinates the transplantation process on-call when a deceased donor organ becomes available
  • Maintains and protects each patient¿s right to confidentiality and adheres to HIPAA guidelines.
  • Answers telephone courteously.
  • Maintains professional demeanor with patients, Physicians, and team members.
  • Other duties as required.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Associates Degree.
  • Minimum 5 years work experience in a healthcare discipline.
  • Minimum of 1 year experience in organ transplant.
  • Minimum of 1 year of experience in an outpatient office setting.
  • Strong organizational and communication skills.
  • Exhibits the ability to work under pressure and use their independent judgment with respect to scheduling conflicts.
  • Solid working knowledge with Microsoft/Google office programs including Excel/Sheets

 

Education, Knowledge, Skills and Abilities Preferred:

  • Experience with clinical assessment and review of solid organ transplant patients

 

Licenses and Certifications Required:

  • Licensure or certification in a healthcare discipline.

 

Licenses and Certifications Preferred:

  • Licensure or certification as a Licensed Practical Nurse or Certified Surgical Technician.
