RN Patient Navigator, Breast Service F/T days
The Patient Navigator, Breast Surg is an advocate and educator for the breast patient, as well as family members through the breast care/cancer continuum.
1. Facilitate communication between patients and healthcare providers. 2. Provides education and educational resources to patients and their families. 3. Supports the patient and family in coping with the psychological aspects of breast cancer. 4. Coordinates appointments with providers for timely delivery of diagnostic and therapeutic services. 5. Refers as appropriate to collaborative care areas. 6. Eliminates barriers for patients to optimize care. 7. Provides community education to raise public awareness.
Education, Knowledge, Skills and Abilities Required: 1. RN, (Associates, Diploma, BSN). 2. 5 years nursing experience in related field (surgery/oncology). 3. Good computer skills. Education, Knowledge, Skills and Abilities Preferred: Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License. Licenses and Certifications Preferred: 1. Certified Breast Patient Navigator in Breast Imaging and Cancer Care.
