The Program Coordinator, CMS Intervention Program grant will help implement and coordinate a public health research project under the supervision of Manisha Parulekar, MD (Principal Investigator). Currently, there is one ongoing research project funded by the Centers for Medicare & Medicaid Services (Hackensack University Medical Center Accountable Health Communities Track 2 Assistance Intervention: Improving Population Health in Bergen County, New Jersey). The Program Coordinator for this grant will play a critical role in ensuring access to health and social care resources for community-dwelling beneficiaries with health-related social needs.

Responsibilities

A day in the life of Program Coordinator, CMS Intervention at Hackensack Meridian Health includes:

  • Support Program Manager in departmental process improvement efforts across clinical delivery sites related to the project. 
  • Coordinate and monitor implementation of all community service navigation services standard operating procedures to ensure program fidelity.
  • Work with program staff to provide community-dwelling beneficiaries with health-related social needs access to resources. 
    Conduct periodic service gap analyses and updating a community resource inventory with partners to assure that resources are available in the community to meet the health-related social needs of beneficiaries. 
  •  Represents the project at various community organization/coalition meetings.
  • Conduct training sessions for screeners and navigators throughout the course of the project.
  • Administer patient surveys, data entry, tracking, and monitoring of patient appointments. 
  • Act as a liaison between hospitals, primary care providers, specialists, community resources and managed care/insurance plans on behalf of patients to ensure person-centered coordination of care.
  • Responsible to ensure that data collection and reporting is accurate and complete.
  • Assist with developing all program progress reports, presentations, and documents.
  • Reports on project activity and progress regularly with the Principal Investigator and Program Manager.
  • Reports and escalates project issues to management as needed. 
  • Assist with supervision and support of assigned staff, which will include Community Health Workers and Interns, and monitoring of navigation cases and appropriate service delivery. 
  • Other duties as assigned by Principal Investigator and/or Program Manager.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor's degree in Human Services, Social Work, Public Health or other social or behavioral science related field.
  • Minimum 2-4 years of social services, human services or community health experience or the equivalent.
  • Requires the ability to travel to multiple office locations; personal vehicle and automobile that is insured. 
  • Demonstrates ability to carefully set priorities, meet deadlines, and schedule time efficiently.
  • Ability to accurately type and effectively operate a computer and use a variety of common software programs including Microsoft Office. 
  • Ability to follow-up and follow-through with strong attention to detail. 
  • Ability to work well and maintain professionalism under occasional times of stress and pressure.
  • Demonstrates ability to follow oral and written instructions.
  • Demonstrates ability to work with other people through a cooperative effort.
  •  Possesses strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, external business partners, and the community.

Education, Knowledge, Skills and Abilities Required:

  • Minimum 1 year staff supervision experience.

Licenses and Certifications Required:

  • Valid Driver's License from a USA state.
