Clinical Coordinator RN, Transfer Center F/T nights
Manages appropriate placement of patients using clinical expertise, critical thinking and demonstrated knowledge of unit specific admission and discharge criteria.
1. Consistently and accurately uses documentation, for example in logs, reports and system notes. Is able to trend and report outcomes, recommend and act on improving outcomes. 2. Maintains documentation relating to current bed status in the facility. 3. Communicates with Access Service team to insure accuracy with patient placement and patient information. 4. Collaborates with nursing units to achieve appropriate patient placement. 5. Demonstrates the ability to handle a high degree of pressure, heavy workloads, multiple requests and numerous interruptions in a positive manner, establishing priorities for effective work completion. 6. Demonstrates expertise in working with multiple computer applications as well as 3 way phone calls and call recording. 7. Arranges transportation with Alert ambulance to facilitate transfers in a timely manner. 8. Utilizes appropriate communication skills in working with their leader, team members, subordinates, medical staff. 9. Demonstrates the ability to independently implement solutions to patient placement or transfer issues. 10. Utilizes critical thinking and decision making processes. 11. Mentors team members. 12. Able to easily diffuse difficult situations and responds appropriately. 13. Performs daily work tasks with extraordinary customer service. 14. Responsible for obtaining and receiving end of shift report. 15. Serves as a clinical resource for the transfer center and patient placement. 16. Responsible for receiving and responding to transfer and patient placement request timely. 17. Ensures appropriate and accurate routing of transfer calls. 18. Documents transfers and patient placement in appropriate computer application for tracking and trending purposes. 19. Follows documented policies and procedures. 20. Maintains current list of physician and nursing on call schedules. 21. Advises Leader of all potential problems and issues in a timely manner. Seeks assistance from the appropriate level of management for complex clinical decisions. 22. Patient Age Specification: Neonate Infant Toddler Pre-School School Age Adolescent Adult Geriatrics
Education, Knowledge, Skills and Abilities Required: 1. RN graduate of an accredited school of nursing. 2. Minimum of 5 years clinical experience in acute care setting.
