Overview
This exciting Nurse Practitioner position will focus on the comprehensive rehabilitation of individuals with various medical and physical impairments who are undergoing acute rehabilitation. Be part of a dynamic interdisciplinary team working together to enhance our patient's functional recovery and return to the community. The position entails comprehensive patient assessment including history, physical and treatment recommendations. The APN will provide leadership and support to the unit, including education, clinical monitoring, and program development. The APN will work in collaboration with a physician and other rehabilitation team members.
We are located in the beautiful suburb of Wayne, NJ in a small community full service hospital with a 20 bed inpatient rehabilitation unit.
While presently a per diem position (flexibility with day time hours) this has great potential to develop into a full time salaried position with benefits.
Must have rehabilitation medicine experience.
Responsibilities
The APN will have the rewarding responsibilities of evaluating and directing the care and recovery of individuals in need of inpatient rehabilitation, and working with a skilled and devoted interdisciplinary team of rehabilitation professionals, medical consultants as well as families with the goal of increasing functional performance, quality of life and ability to return to the community.
Primary responsibilities for a 20 bed inpatient rehabilitation unit housed in an acute care hospital under the supervision of a physiatrist:
Performing inpatient admissions in all aspects, as well as transfer and discharge summaries
Excellent working knowledge of electronic records including electronic prescription
Sound knowledgeable of medicine
Ability to interact well with the rehabilitation team including PT/OT/ST, nursing, and SW
Ability to work well with a team of consultants in all medical specialities as well as families
Advocacy and appeals for inpatient rehabilitation patients to insurance providers when needed
Full time position with desire for long term commitment
Desire to work in a team with primary goal of optimal quality patient care
Qualifications
Desire to work in a team with the primary goal of optimal quality patient care
Ability to work well with a team of medical consultants and coordinate patient care at all levels
Ability to interact with, supervise and lead the interdisciplinary rehabilitation team including PT/OT/ST
Sound knowledge of medicine
Excellent working knowledge of EMR
Presently inpatient with some flexibility for outpatient rehabilitation experience based on interest and need
Education, Knowledge, Skills and Abilities Required:
1. Graduate of an NLN/AACN accredited program in nursing.
2. Master's degree.
3. Maintains clinical competencies through participating in continuing educational programs (contact hours per years), and clinical practice as mandated by Board of Nursing and national certification body.
4. Experience in relevant patient care setting or as regulations dictate.
5. In collaborative agreement with physicians on staff- fulfills terms of collaborative agreement.
Education, Knowledge, Skills and Abilities Preferred:
1. A minimum of 3 years advanced nursing experience.
2. Graduate of an accredited School of Nursing and an Accredited Nurse Practitioner Program (Masters).
3. Cerner EMR knowledge.
Licenses and Certifications Required:
1. New Jersey Advance Practice Nurse License, CDS and DEA
2. AHA Basic Health Care Life Support HCP Certification.
3. Active NJ RN license.
