Responsibilities

A day in the life of a Supervisor for RN Care Coordination at Hackensack Meridian includes:

  • Supervising assigned staff to ensure that all care coordination processes are in order; including coordinating patient activities to ensure that care meets evidence-based practice standards, and regulatory/payor requirements. The role integrates and coordinates utilization management, care coordination, discharge planning functions and performance improvement activities for the care coordinators. 
  • Accountable for the department’s designated staff caseload and plans effectively in order to meet staffing needs, manage the length of stay, and promote efficient utilization of resources; specific functions within this role include:
    Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery.  Maintains an assignment for direct care coordination with patients and their families.
  • Application of process improvement methodologies in evaluating staff’s documentation.
  • Coordination of communication with physicians, nursing, and staff of ancillary departments.
  • Provides assistance for handling difficult, unusual or emergency situations in order to minimize financial, legal or risk management liability for the facility.
  • Communication with patients and their families around medical plan of care and discharge plan, in difficult cases.
  • Supervises the staff with all care coordination processes, including LOS, throughput, patient flow and readmissions.
  • Responsible for staff productivity and ongoing education.
  • Responsible for staff performance evaluations, disciplinary action and performance improvement plans when appropriate in conjunction with the Director of Case Management.

Qualifications

  • Current and valid RN license in the State of New Jersey required
  • 5 years of case management/care coordination experience in an acute care setting required
  • Supervisory experience preferred
  • Excellent interpersonal communication and negotiation skills
  • Strong analytical, data management and PC skills
  • Current working knowledge of discharge planning, CMS regulations, care management, performance improvement, and managed care reimbursement
  • Understanding of pre-acute and post-acute venues of care and post-acute community resources
  • Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components
  • Ability to work independently and exercise sound judgment in interactions with physicians, payers, and patients and their families
