The Intake Specialist obtains patient information via telephone interviews with patients, their families or other referral sources. Consults with clinical staff to provide a provisional level of care (LOC) recommendations and schedules patients for that LOC evaluation. Enters intake information into the computer system and makes every effort to provide the patient with a smooth and efficient entry into Carrier Clinic.
Responsibilities
A Day in the life of an Intake Specialist at Hackensack Meridian includes:
- Answers all telephone inquiries promptly and provides pertinent information regarding inpatient services to patients, families and referral sources.
- Enters accurate and complete patient data (demographics, insurance, and clinical information) into the computer system.
- Triages patients for level of care in consultation with clinical staff.
- Schedules patient appointments and provides assistance in all aspects of patient intake.
- Cross-trained to perform registration, financial counseling, and other tasks when needed.
- Requests medical records and other clinical information from professional referral sources prior to patient's evaluation.
- Initiates patient call backs.
- Provides reassurance and support to the patients, families and friends in order to reduce anxiety levels associated with the admissions process.
- Serves on hospital committees as appointed.
- Follow up with patients to complete paperwork not signed during the admissions process.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- HS diploma or equivalent.
- Excellent written and verbal communication skills.
- Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Associate's or Bachelor's degree from an accredited college/ university in social work, psychology, nursing or related field.
- One year behavioral health care experience.
